HOW TO IMPORT RECORDS v4.x from Microsoft ® EXCEL,
Microsoft ACCESS, or SYLK format

- Open the LASIK, PRK, and Phakic IOL Outcomes Analysis Software program and have it running on the screen.
- Go to the OPTIONS menu.
- Click the Import from Excel button.
- Follow the on screen instructions to locate the file you wish to import.
- Your Outcomes Analysis Software file will be automatically updated with the records you had in your spreadsheet.
- If you see the message that the records have been imported, but when you look at the database, you fail to fine any imported records, you have most likely not properly placed data in the appropriate columns or placed incorrect data in a given column (see below).
VERY IMPORTANT!
Before importing data, the most important thing to be sure of is what each column in your spreadsheet or database represents.
To place the fields from your spreadsheet of your existing data into the proper fields in the LASIK, PRK, and Phakic IOL Outcomes Analysis Software program, you must know what each column contains and place them in the specific order listed below. To facilitate the import, we provide you with a skeleton Excel spreadsheet with the columns labeled.
The first field must be in your column A (1), the second field must be in your column B (2), the third field must be in your column C (3), etc. If you do not have data to import into a field, LEAVE THAT COLUMN BLANK. When you have finished creating your columns, there should be all columns total with each row representing an eye's record from your database (see the import list below).
If you do not have data for a particular column, leave it blank, BUT DO NOT DELETE A COLUMN or else your data will be imported into an incorrect field.
Be sure to enter right or left for the eye operated (NOT OD or OS) as the program will not recognize the letters and the imported data will look garbled.
If you are using the Microsoft Excel 2000™ program and the Outcomes Analysis Software program does not recognize it, or the import brings in garbled data into the fields in your program, while in Excel, save your files in SYLK format instead of Excel format and retry the import.
You will not need label the columns in the spreadsheet that is to be imported as the column labels will be imported and make at least one incorrect record as long as you use our skeleton spreadsheet.
Calculation fields are not imported. If you have the Flat K and Steep K readings, they will be imported and the program will calculate the Mean K for you.
Each record is unique and so must have a unique record number. To prevent duplication of record numbers, a protection override has been written for birth dates to avoid duplicate birth dates. Be sure to use the column for a birth date for each record you wish to import to avoid the program's "hanging up" once records are imported.

NOTE
If you do not use the diagnosis and surgical procedure words EXACTLY as written above, the automated searches will not work. You will, however, be able to use the custom-find screens.
